Our RNs can oversee even the most complex situations

Sometimes clients have complex health needs that require special care management. In these instances, Health People assigns a registered nurse to oversee the client’s care. While managing care, our RN often will coordinate with a hospital, multiple doctors, and a physical therapist. Our RN will schedule appointments, accompany the client to medical appointments, and handle all details associated with the client’s health care.

Care Management services include:

Coordinate communication among doctors, physical therapists, attorneys and trust officers, and family members
Review and manage medication
Assess Medicare and insurance benefits
Facilitate resources and services
Step in as the single point of contact for the client’s care management
